New Practical Pathway Launched TO Support Early Identification And Management Of Frailty, Sarcopenia And Malnutrition

A new evidence-based Frailty, Sarcopenia & Malnutrition Practical Pathway has been launched to help health and care professionals more effectively identify and manage three overlapping conditions that significantly affect the wellbeing and independence of older adults.

Frailty, sarcopenia and malnutrition frequently coexist yet remain under-recognised in many clinical settings. Early identification and timely, targeted intervention can make a meaningful difference to outcomes – reducing avoidable decline and supporting people to live well for longer.

Developed and reviewed by clinicians with extensive experience working with older adults, the new pathway provides a clear, practical framework for understanding the key factors that contribute to frailty and its progression. It brings together the essential components of good practice, including:

  • Early identification and assessment
  • Recognising observational indicators
  • Using validated clinical tools for frailty, sarcopenia and nutritional risk
  • Dietary strategies to support adequate intake, including oral nutritional support where appropriate
  • Guidance on physical activity and resistance exercise to maintain or improve strength and function
